Sworn or affirmed statement

Personal details recorded in the Municipal Personal Records Database (Gemeentelijke Basisadministratie Persoonsgegevens, GBA) must be taken from official source documents. The details in the GBA have to be correct, complete, up to date and reliable.

When a sworn statement is needed

Living - Official Matters - Sworn Statement 

If it is not possible to supply official source documents, you can make a sworn or affirmed statement at the municipality.

In this case, the necessary details are taken from the sworn or affirmed statement made in the presence of an official at the Department of Civil Affairs (Burgerzaken) of an urban district council office (stadsdeelkantoor).

Conditions

In order to be eligible to make a sworn or affirmed statement you have to be at least 12 years of age. Those under the age of 12 must be accompanied by a parent, guardian or carer.

Costs

There are no costs for making a sworn statement.

Application

You can make a sworn or affirmed statement at the Department of Civil Affairs (Burgerzaken) of an urban district council office (stadsdeelkantoor). You have to make an appointment in advance regarding the time the statement will be made. If necessary, arrangements can be made to have an interpreter present.

Documents to take along

When you are going to make the statement you need to take all official documents you have in your possession, including the ‘report of follow-up interview’ (‘rapport van nader gehoor’).

From a country outside the Netherlands

If you wish to take up residence in the Netherlands from abroad you first have to register with the Personal Data Department, (Dienst Persoongegevens), Immigration and Nationalities Division (Afdeling Immigratie en Nationaliteiten). For that purpose you need to make an appointment by phone. When making the appointment indicate that you do not have any source documents. A sworn statement will then be taken from you at the same time that you register.
